See What a Measure Choice Actually Does to Your Score
eClinicalWorks can track which quality measures you're reporting and your numerator and denominator counts. What it can't do is run those numbers through CMS's actual scoring formula, or tell you what your final score means for next year's Medicare payment.
The negative side of that formula is fixed in statute: a linear scale down to a 9 percent cut at a final score of zero. The positive side isn't fixed anywhere until after the performance year closes. This calculator applies the real math to the first and models the second as an adjustable scenario, not a guess dressed up as a guarantee.
What's Inside This Calculator?
One workbook, three tabs: the 2026 scoring rules, a place to build each category's score, and the final number with a dollar figure attached.
1. MIPS Scoring Reference
- The 75-point performance threshold and all four category weights come prebuilt for 2026
- Small practices that skip Promoting Interoperability get the automatic reweighting CMS actually applies, not a simplified guess
- The statutory 9 percent negative cap is built into the math, sourced and cited
- A default positive-adjustment scaling factor is included, with the real historical range next to it so you know it's a placeholder
2. Category Score Builder
- Up to six Quality measures, each scored the way CMS actually scores them, including the 3-point floor for small practices that miss data completeness
- Improvement Activities points calculate from how many high- and medium-weight activities you attest to, doubled for small, rural, or HPSA practices
- Cost and Promoting Interoperability come in as direct percentages, since both are calculated entirely from claims and certified EHR data no outside calculator can replicate
- Every input ties back to a specific CMS rule, not a rounded estimate
3. Final Score & Payment Impact
- Your weighted final score rolls up automatically across all four categories
- A Category Improvement Priority table shows exactly which category has the biggest point gap left to close
- Enter your Medicare Part B allowed charges, and the projected payment adjustment turns into an actual dollar figure
- Change one input and watch the score, the adjustment, and the dollar figure all move together
