E/M Services Clarifications: The CPT Editorial Panel addressed clarifications sought by the Centers for Medicare and Medicaid Services (CMS) related to the reporting of evaluation and management (E/M) services. Changes include the removal of time ranges from office or other outpatient visit codes (99202-99205, 99212-99215) and alignment of the format with other E/M codes. Additionally, a definition to determine the “substantive portion” of a split/shared E/M visit has been provided, offering guidance when physicians and non-physician practitioners collaborate to provide comprehensive care. Instructions for reporting hospital inpatient or observation care services and admission and discharge services have also been added, specifically for the use of codes 99234-99236 when a patient’s stay spans two calendar dates.
